PRODUCT RECALL LAWSUITS RELATING TO CONTAMINATION OF TRIAD ALCOHOL SWAB AND PAD AND OTHER TRIAD PRODUCTS

Manufacturing Defects in products manufactured by Triad Group of Hartland, Wis. have led to recalls of alcohol prep pads and swabs due to bacterial contamination of these and other products the company manufactured. Their products contain a dangerous bacteria known as Bacillus Cereus. This bacteria can cause serious and potentially life-threatening infections and are alleged to have led to brain damage in at least one infant exposed to these products in a Neonatal Intensive Care unit. Bacillus Cereus infections are likely underreported because unless the bacterium is specifically cultured, it doesn't show up in test results.

What Brands or Labels appear on Triad products?

Triad wipes are sold over-the-counter and sold in the United States, Canada, and Europe. H&P Industries, which runs Triad Group, sells generic medical products under private labels at common grocery and drug store chains including grocery stores such as Safeway and Krogers and drugstores such as CVS and Walgreens. The names Cardinal Health, Conzellin, PSS Select, VersaPro, Boca/Ultilet, Moore Medical, Walgreens, and/or CVS may appear on the product packaging. Although the wipes have been recalled, some internet retailers still have them available for purchase. FDA inspection documents also show possible contamination problems with other Triad products that have not been recalled such as sterile lubricating jelly, mouthwash, and cold medication.

Investigation

Since February, the Triad Group and H&P Industries Inc. have been the subject of an ongoing msnbc.com investigation that found that lax government oversight and repeated violations of good manufacturing practices allegedly led to infections and death tied to tainted wipes and pads. Contamination of Triad products with this organism was confirmed by federal Food and Drug Administration investigators last year. This led to massive recalls of the alcohol prep pads in January, seizure of more than $6 million in Triad products in April and, in June, a federal court order barring the Wisconsin plant from making and distributing medical supplies. Records show since January, federal Food and Drug Administration officials have received reports of eight deaths, 11 potentially life-threatening infections and nearly 250 other problems associated with Triad alcohol and povidone iodine prep productsand sterile lubricating jelly. FDA officials are investigating two of the deaths that specifically cite Bacillus cereus.
